This Notice of Privacy Practices (“Notice”) details your rights and our obligations regarding the use, protection, and disclosure of your Protected Health Information (“PHI”).
Our Commitments
- Confidentiality and Privacy: We are legally obligated to protect the privacy of your PHI and provide this Notice explaining our privacy practices.
- Notice Revisions: This Notice is effective as of the date listed here and remains valid until any amendments are made. We reserve the right to change our privacy practices and this Notice according to changes in applicable law and our operational practices. Any new Notice will be made available upon request, through our website, and will be posted in our facilities.
Use and Disclosure of PHI
- Treatment and Operations: We may use or disclose your PHI as necessary for your treatment and to run our healthcare operations. For example, your PHI may be shared with healthcare professionals involved in your care, or used for billing and administrative purposes.
- Emergency Situations: In emergencies, we may disclose your PHI to medical personnel involved in your care.
- Legal Requirements and Law Enforcement: We may use or disclose PHI when required by law, including court orders or in response to legal investigations.
Your Rights Concerning PHI
- Access to Your PHI: You have the right to access and obtain a copy of your PHI that we maintain, with certain exceptions defined by law.
- Amendments to Your PHI: You have the right to request an amendment to your PHI if you believe it is incorrect or incomplete.
- Accounting of Disclosures: You are entitled to request an accounting of certain disclosures we have made of your PHI.
- Restrictions on Use: You may request restrictions on certain uses and disclosures of your PHI. We are not required to agree to these restrictions, but if we do, we will abide by them unless it is necessary to provide emergency treatment.
- Confidential Communications: You have the right to request that we communicate with you about medical matters in a more confidential way, such as sending mail to an address other than your home.
Special Protections for Substance Abuse Records
Federal law provides special protections for records of the identity, diagnosis, prognosis, or treatment of any patient maintained in connection with the performance of any program or activity relating to substance use disorders. Such records may only be disclosed for the purposes specified in the regulations and with your explicit written consent or by a specific court order.
Breach Notification
In the event that there is a breach of unsecured PHI concerning your health information, you will be notified as required by law.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the Secretary of the U.S. Department of Health and Human Services. All complaints must be submitted in writing. You will not be penalized or retaliated against for filing a complaint.

It is effective as of August 8, 2024 and remains in effect until amended or replaced. You are entitled to receive a copy of this notice at any time.
